Fracture Description
1 . Name of Injured Bone
2 . Integrity of Skin/Soft Tissue
Displacement
Refers to position of the distal fragment relative to the proximal fragment
Varus/Valgus Angulation
Varus = Apex away from midline Valgus = Apex toward midline

“Thumbs Up”: PIN (Radial Nerve) “OK Sign”: AIN (Median Nerve) “Spread Fingers”: Ulnar Nerve
X-Ray Rule of 2s
2 sides = bilateral
2 views = AP + lateral
2 joints = joint above + below
2 times = before + after reduction

environment, or contaminated (i.e. bowel)
